Medical insurance is offered through United Healthcare and is effective on the employee date of hire. Medical coverage options consist of a $0 deductible plan, a $500 individual/$1,000 family deductible plan or a $1,500 individual/$3,000 family high deductible plan that qualifies for an HSA account. All three plans utilize United’s PPO Network. Ampirical pays the same set dollar amount for employee and dependent coverage regardless of the plan that is chosen.
Dental insurance is offered through the Principal Financial Group, effective on the first of the month following employee’s date of hire. Ampirical pays 75% of the premium for each full-time employee and all dependents.
Vision insurance is offered through the Principal Financial Group, effective on the first of the month following employee’s date of hire. Ampirical pays 75% of the premium for each full-time employee and all dependents.
Life insurance coverage is supplied to each full-time employee at no cost to the employee, effective on the first of the month following date of hire. Additional Voluntary Life is also available through Principal.
Optional supplemental life and health insurance is available to each full-time employee through the AFLAC Group.
Long-term disability (LTD) insurance is supplied to each full-time employee at no cost to the employee through Principal Financial Group.
Short-term disability (STD) insurance is supplied to each full-time employee at no cost to the employee through the Principal Financial Group. Coverage is based on 60% of salary, with a maximum of $6000 per month. STD benefits begin after the 7th calendar day of employee’s inability to work and continues until long-term disability goes into effect.
401K is offered through American Funds. Company match is $1.00 for every $1.00 up to a maximum of 3% and $0.50 per $1.00 for the 4th and 5th % (4% total match on 5% employee contribution). 401K benefits begin on the first day of the month following date of hire. Employees are 100% vested immediately. Employees may also contribute to a Roth Elective Deferral.
Direct deposit of payroll is available for all employees including direct deposit of funds into multiple accounts, i.e., 401K, Health Savings Accounts, LA Start (529), and Employee Personal Checking and Savings Accounts. Current employees have access to all paystubs (past and current) through Netchex Online.
Sick pay is offered to all full-time employees up to 40 hours per year (pro-rated based on hire date). Effective January 1st of each year all eligible employees shall receive 40 hours of sick time. The amount of sick time provided to new hires will be based on the following schedule: First Quarter Hire – 40 Hours, Second Quarter Hire – 30 Hours, Third Quarter Hire – 20 Hours and Fourth Quarter Hire – 10 Hours.
Vacation pay is provided to all full-time employees up to 80 hours effective January 1st of each year. Vacation shall accrue at the rate of 3.077 hours per pay period (x 26 pay periods).
Holiday pay is provided to all full-time employees for 10 paid holidays per year.
Pay periods occur every other Friday.
A flexible work schedule is offered to all employees, including the option of a work schedule that allows an employee to take every other Friday off, as well as flexibility in scheduling their arrival and departure times.
A Sam’s Club Corporate Membership is offered on an optional basis to all full-time employees. Corporate membership allows employees to shop during early morning hours prior to access by the general public. Sam’s Club Membership also offers travel and other discounts to its members.
Tuition Reimbursement is available to all full-time employees who have reached 12 months of tenure with Ampirical and who are not in any type of probationary status. Certain restrictions apply and are set forth in the firm’s Tuition Reimbursement Policy.
A gym club sponsorship is given to all full-time personnel. All full-time employees are eligible to receive a stipend of up to $24.00 per month to be applied to employee’s gym membership dues.
